Case Management

Case Management services are provided to families of children, youth and adults with developmental disabilities. Assistance is provided which links consumers to needed entitlements such as, SSI, Medicaid, Public Assistance and educational services. Referrals are also made for camp, respite, counseling, family reimbursement, evaluations and other support services. Non-Medicaid eligible families are also served. Eligible families must be residents of Manhattan, Bronx, Brooklyn or Queens.

East Harlem Neighborhood Based Alliance (EHNBA)

Case Management services are provided to families residing in East Harlem and the Upper Harlem Communities to connect and link them to needed services.  Parent support groups are also available in spanish.  Please see calendar of trainings for schedule times.
